Georgia Report of Divorce, Annulment or Dissolution of Marriage

Form 3907 (Rev. 10-2002) PDF was revised effective January 2, 2003, for the purpose of reporting information about all divorces. The Georgia Code requires a record of each divorce, annulment or dissolution of marriage to be filed with the Department. To facilitate the collection of the data, the form was revised to only report divorce information; case registry questions have been removed.

Attorneys:
This form is to be completed by the attorney and filed with the Superior Court Clerk at the time of the disposition. OCGA 31-10-22 states. "In all cases, the completed report shall be prerequisite to the granting of the final decree."
