Selecting “Application Packet” will download all of the files needed for filing an application to the Georgia Department of Public Health Division of Public Health Dietetic Internship. Individual portions may be downloaded by selecting a specific file name listed below. The application form and recommendation form are writable and may be completed, printed, and saved to your computer.
