- The Division of Public Health, Epidemiology Branch, Immunization Program and Health Districts collaborate on the Georgia Immunization Study. The purpose of the study is to assess the immunization coverage rates of two-year-old children in Georgia statewide and for each of the eighteen health districts.

(form 3184)

- Health care providers may use this request form to order: Certificates for School / Child Care Attendance; Parent and Child Education Materials; Immunization Record Forms; Report and Request Forms; and Vaccine Information Statements.

Sample Immunization Certificate (form 3231)

- This form is a sample only. Only health departments and physicians licensed in Georgia can obtain blank immunization certificates (Form 3231). Take your child's personal immunization record to a health department or Georgia physician and they can complete the form and give any required vaccines.
