Maternal and Child Health (MCH) Title V Block Grant
Information for Public Health Staff
This web page has been designed to inform public health staff about the performance measures
and also to encourage district and state level public health staff to report on activities supporting
the performance measures.
The performance measures are divided into two sections: National Core Measures, the priorities
that are federally required, and State Performance Measures, the priorities that the Georgia Department of Public Health has developed. You can read about the activities that have been submitted in the
FY 2005 MCH Block Grant Annual Report as well as the FY 2006 plan in the
Georgia Block Grant Narrative.
If during the current fiscal year (July 1, 2005 to June 30, 2006), you are involved with an activity
that supports one or more of the Performance Measures, you may add your activity to the Annual Report
by emailing the following information to mchblock@dhr.state.ga.us:
- National and state performance measures.
- Activity addressed (short descriptive title);
- Specific details of supporting activity (two or three sentences);
- Status of activity:
- pre-planning
- in progress
- completed
- projected date of completion
- Partners;
- Contact name, job title, phone number and e-mail address; and
- District, county, or state office.
Here is an example of how you can submit an activity:
- National and state performance measure;
SPM 10: The extent to which partnerships that support Early Childhood Comprehensive Systems (ECCS) are effective
- Activity addressed;
Asthma education in Example County Schools
- Specific details of supporting activity (two or three sentences);
Example County Health Department provided a "lunch and learn" educational opportunity for the five school nurses employed by the Example County Schools. The treatment of children 4-18 years of age with asthma was discussed, with a particular focus on the schools.

We anticipate that this electronic MCH Performance Measure Reporting process will assist us in
collecting more specific and accurate information for the MCH Block Grant about the successful
activities and programs for mothers and children that are occurring throughout the State. In addition,
we expect that this process will also increase awareness internally of the myriad of activities
that are planned and implemented in support of the National and State Performance Measures.
