Diabetes Control Program

Mission

The mission of the Diabetes Control Program is to educate diabetes patients to control their disease and minimize the risk of complications from the disease.

The Georgia Diabetes Control Program provides professional education and patient education and management services. Services are provided to persons with diabetes in seven public health districts (Rome, Columbus, Albany, Valdosta, Waycross, Brunswick and Dublin).

The program offers:

  • education
  • nutrition services
  • monitoring to prevent, delay or detect early complications
  • laboratory tests such as blood glucose and cholesterol testing
  • blood pressure measurement
  • foot exams

The education provided covers the basic skills and knowledge required for self-care and management to prevent complications of diabetes. In the Gainesville District, professional education and classes for persons with diabetes in the community are provided. Limited services are available in other health districts that do not have formal Diabetes Control Programs. Further information on patient services is available from local county and district health offices. State Program staff and contracts with the Medical College of Georgia and with Grady Memorial Hospital provide education to health professionals statewide.
