Fiscal Year 2006 (July 1, 2006 - June 30, 2007)

Percent of infants and toddlers with IFSPs who:

  • Indicator 1. receive the early intervention services on their IFSPs in a timely manner. The state target is 100%.

  • Indicator 2. primarily receive early intervention services in the home or programs for typically developing children.

  • Indicator 3a. demonstrate improved positive social-emotional skills (including social relationships)
  • Indicator 3b. demonstrate improved acquisition and use of knowledge and skills (including early language/ communication)
  • Indicator 3c. demonstrate improved use of appropriate behaviors to meet their needs

Percent of families in Birth to Three for at least six months who report that early intervention services have helped the family

  • Indicator 4a. know their rights
  • Indicator 4b. effectively communicate their children's needs
  • Indicator 4c. help their children develop and learn

  • Indicator 5. Percent of infants and toddlers age birth to 1 on 12/1/07 with IFSPs

  • Indicator 6. Percent of infants and toddlers age birth to 3 on 12/1/07 with IFSPs

  • Indicator 7. Percent of eligible infants and toddlers with IFSPs for whom an evaluation and assessment and an initial IFSP meeting were held within 45 day from referral.

Percent of all children exiting Part C who received timely transition planning to support the child’s transition to preschool and other appropriate community services by their third birthday including:

  • Indicator 8a. IFSPs with transition steps and services
  • Indicator 8b. notification to school district if a child is potentially eligible for preschool special education
  • Indicator 8c. a transition conference at least 90 days before age 3, if the child potentially eligible for preschool special
    The state target is 100%.
